That’s why Roane General Hospital has developed a multi-layered programa program that will be personalized just for you. We will provide you with a Prescription for Your Health and accompany you on your way to wellness in a supported environment.
Who Can Participate?
While Prescription for Your Health is especially beneficial to those with a chronic disease or those with multiple health risk factors, any active Roane General Medical Clinic patient that is interested in improving their health is eligible to participate. Other members of the community may also join the program with referrals from their Primary Care Providers.

Prescription for Your Health starts with your Primary Care Provider (PCP), who will provide medical oversight in the development of a personal plan of care. The plan will include goals for improved health factors and a schedule for preventive screenings and wellness checks.
In addition to your PCP, you will work with health coaches and fitness trainers to support your plan. Our health coaches provide health education, while also being a point of contact on a quarterly basis to help monitor progress toward goals. Our fitness trainers work to improve your overall level of fitness in a safe and supervised environment.
Access to Health Education
Health literacy is essential to decisions you make about the health services you receive, the ability to manage chronic conditions at home, and the overall ability to maintain your personal health and wellness. To support health literacy, Prescription for Your Health participants have free access to the following health and nutrition education classes at Roane General Hospital.
- Basic Nutrition
- Healthy Cooking
- Diabetes Management
- Tobacco Cessation
- And more!


Free Access to Medical Fitness
Prescription for Your Health participants will receive an individualized fitness program and have it supervised by one of our qualified staff. Participants will gain free gym access and free entry into any of our group fitness classes. This is an important part of the program as physical fitness is a crucial part of maintaining good health and wellness. Regular exercise and physical activity promotes strong muscles and bones. It will also improve respiratory, cardiovascular, and overall health. Staying active can also help you maintain a healthy weight, reduce your risk for type 2 diabetes, heart disease and some forms of cancer.

Our Prescription for your Health programs give you the guidance you need to find your way to wellness.
Prescription for Your Health is comprised of three main components:
- PCP Health Plan
- Nutrition and Health Education
- Medical Fitness
PCP Health Plan
As part of your normal Primary Care Provider (PCP) visit, you are presented with a Care Plan. This document is developed with you and your care team to outline your health and wellness goals.
Prescription for Your Health participants will be able to utilize our education and fitness services to assist them in meeting their health and wellness goals.
Health Education
Health literacy is essential for successful access to care and use of services, self-care of chronic conditions, and maintenance of health and wellness. Health literacy is fundamental to healthcare that requires individuals to have a more active role in decisions and management.
Prescription for Your Health participants are able to take advantage of health education classes offered by Roane General Hospital free of charge. These classes include, but are not limited to: Basic Nutrition, Healthy Cooking, Diabetes Management, and Tobacco Cessation.
Medical Fitness
There are numerous health advantages to physical fitness. Regular exercise and physical activity promote strong muscles and bones. It improves respiratory, cardiovascular, and overall health. Staying active can also help you maintain a healthy weight, reduce your risk for type 2 diabetes, heart disease, and reduce your risk for some cancers. In other words, staying active is a crucial part of maintaining good health and wellness.
Prescription for Your Health participants will create an individualized fitness program with one of our qualified staff. Participants will gain free gym access and free entry into any of our group fitness classes.
Program Incentives
Participants in Prescription for Your Health with receive multiple benefits for compliance with their contract. In order to receive the benefits participants must present an active Benefit Card.
- Free use of RGH Fitness Complex and access to group classes
- Free enrollment in all education classes / support groups
- Annual Health Fair laboratory testing fee waived
- 20% Discount at Healthy Way Café
Participation
Current active Roane General Medical Clinic patients in good standing are eligible for the Prescription for your Health Program. Participants may sign up once every 12 months.
Participants will meet with the Prescription For Your Health (P4YH) staff when signing up to complete their contract and to be issued their benefit card.
Compliance
Program participation will be reviewed every 90 days. Participants in good standing will receive a new benefit card.
Patients who fall out of compliance after a 90 day review will be able to meet with the P4YH health-coach to discuss ways to help them meet their goals and the terms of the agreement.
Two consecutive 90 day reviews out of compliance will terminate the participant from the program and all program incentives will cease.
Termination of Program Participation
Individuals who no longer wish to participate in the Prescription for Your Health program need to notify the Prescription for Your Health team to be removed from the program. When removed from the program all Program Incentives will cease.
